Job summary

Hospice of North Alabama is seeking a dedicated LPN to join our team serving Decatur, Moulton, Hillsboro and Trinity in Alabama. You will provide 1:1 patient care in a hospice setting, following the care plan and state nursing rules, and collaborate with the hospice team.

The successful candidate will have an active LPN/LVN license in Alabama, CPR certification, and strong communication skills. This role offers a stable daytime schedule with growth opportunities and a supportive environment.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the state.
• CPR certification current.
• Own a vehicle with insured automobile in good standing.
• Demonstrate good verbal and written communication.
• Understand hospice philosophy and terminal illness needs.

Responsibilities

• Give hands-on care to patients nearing end of life following care plan and state rules.
• Follow the care plan after assessing patient needs.
• Keep clear, timely records of care provided.
• Educate patients and families about illness and care plan.
• Coordinate care by communicating with the hospice team.
• Manage pain and symptoms and assess treatment effectiveness.
• Participate in training and quality improvement to grow professionally.

As a Hospice LPN You will:

• Give hands-on care to patients nearing the end of life, following state nursing rules.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification.
• Understand philosophy of hospice concept. Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
