Job summary

Adoration Health is seeking a Licensed Practical Nurse to join our Home Health team in Lexington, KY. This role provides 1:1 patient care in the home, following each client’s plan of care while maintaining dignity and independence.

You will administer meds, feeds, oxygen, and ostomy care, documenting progress and coordinating with families and physicians. We offer full-time, part-time, or PRN schedules, with benefits including medical/dental/vision, HSA/FSA options, tuition assistance, a 401(k)

Qualifications

• LPN/LPT/LVN license in good standing in state of practice.
• CPR certification required.
• 1+ years nursing experience preferred; home health experience preferred.

Responsibilities

• Provide 1:1 care per client’s plan of care.
• Administer medications, feedings, oxygen, ostomy care per plan and document outcomes.
• Document nursing actions and communicate with family, case manager, and physicians.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
