Job summary

Adoration Health, part of BrightSpring Health Services, is hiring a Home Health Licensed Practical Nurse for Lexington, KY. The role provides 1:1 patient care in home settings, with a focus on maintaining patient dignity and independence under a patient-care plan.

You will administer meds and therapies, document actions, and collaborate with families and care teams. Full-time, part-time, or PRN options offer flexibility and growth within a growing home health team.

Qualifications

• Current unrestricted nursing license in the state of practice.
• Valid driver’s license with proof of insurance.
• CPR certification.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care to follow the plan of care and maintain patient dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per plan of care.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings as needed to ensure availability.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
