Adoration Health is seeking a Home Health Licensed Practical Nurse to join our Anmoore, WV team. You will provide individualized, compassionate in-home care under the patient’s care plan and collaborate with family and case managers to monitor progress.

This full-time role offers stable hours, independence with team support, and opportunities for professional growth within a growing organization.

Qualifications

• LPN/LPT/LVN Degree from an accredited nursing program with active license in state of practice
• CPR certification required for clinical duties
• 1+ years of nursing experience preferred, home health experience preferred

Responsibilities

• Providing 1:1 client care per plan of care while maintaining dignity and independence
• Administering medications, feedings, oxygen, and ostomy care per plan and assessments
• Documenting nursing actions and communicating with family, case manager, and physicians
• Coordinating or ordering supplies, medications, and feedings to ensure adequate stock
• Ensuring patient needs are met per physician instructions

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
