Job summary

Adoration Home Health is seeking a licensed practical nurse to provide one-on-one home health care in Glasgow, KY and surrounding areas. The role involves administering medications and treatments per the care plan, documenting progress, and communicating with families and the care team.

Ideal candidates have an LPN/LPT/LVN license, CPR certification, a valid driver's license, and at least 1 year of nursing experience, preferably in home health.

Qualifications

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and residence.
• CPR certification required/maintained.
• 1+ years nursing experience preferred, with home health care experience preferred.
• Valid driver's license with acceptable driving record and proof of car insurance.

Responsibilities

• Providing 1:1 home health care aligned with the client’s plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed and assess results.
• Document nursing actions, progress, and communicate changes to physicians and care team.
• Coordinate or order supplies, medications, and care items to ensure availability.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
