Job summary

Adoration Home Health is seeking a Licensed Practical Nurse to join our Searcy, AR team as a Home Health LPN. This full-time role focuses on 1:1 patient care in the home setting, following the care plan, with opportunities for growth within a supportive, compassionate environment.

The position requires a valid LPN/LPT/LVN license, CPR certification, and at least 1 year of nursing experience (home health preferred). A driver's license and reliable vehicle are required.

Qualifications

• LPN/LPT/LVN with active license in the state of practice.
• Valid driver’s license and proof of car insurance.
• CPR certification.
• 1+ years of nursing experience; home health care preferred.
• Ability to work independently with team support.

Responsibilities

• Provide 1:1 care per client’s plan while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care per plan.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings to ensure availability.
• Meet patient needs per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
