Job summary

Adoration Health is seeking a Home Health Licensed Practical Nurse (LPN) for PRN shifts in Crossville, TN. You will provide 1:1 care following each client's plan of care, helping maintain dignity and independence.

Responsibilities include administering medications, documenting nursing actions, coordinating supplies, and communicating with families, case managers, and physicians. Prior home health experience and CPR certification are preferred.

Qualifications

• LPN/LPT/LVN degree with current, unrestricted license.
• Valid driver’s license and proof of car insurance.
• CPR certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care per client's plan while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings as needed to ensure supplies are on hand.
• Ensure all patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
