Job summary

Adoration Health in Florence, SC seeks a licensed practical nurse (LPN) to join our Home Health team. You will provide 1:1 patient care at home per each plan of care, administer medications, document care, and coordinate supplies with the care team.

This full-time role offers a $5k sign-on bonus and opportunities for advancement within a growing company. Qualified candidates will have an LPN/LPT/LVN license, CPR, a valid driver’s license, and 1+ year of nursing experience; home health experience

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice
• Valid driver’s license with acceptable driving record
• CPR certification in good standing
• 1+ years of nursing experience preferred
• Home health care experience preferred

Responsibilities

• Provide 1:1 home care following the client’s plan of care while preserving dignity and independence
• Administer medications, feedings, oxygen, ostomy care as required and document results
• Accurately document nursing actions and communicate with family, case manager and physicians
• Coordinate or order supplies, medications, and equipment to ensure adequate stock
• Ensure patient needs are met per physician instructions

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
