Job summary

Amedisys is seeking a Licensed Practical Nurse (LPN) to join our home health team in Crestview Hills, KY. This full-time position involves on-site visits to patients in Kenton County, collaborating with RNs and physicians, and delivering compassionate, patient-centered care.

Responsibilities include verifying plans of care, teaching self-care, monitoring vital signs, and performing medication reconciliations.

Qualifications

• One year of clinical LPN experience.
• Current CPR certification.
• Valid driver’s license with reliable transportation.
• Current LPN license, state(s) you are assigned to work.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
