Job summary

Adoration Health is seeking a Licensed Practical Nurse for Home Health in Colonial Heights, VA. This full-time role involves delivering patient-centered, 1:1 care in the home, following each client’s plan of care, and working collaboratively with a supportive team.

Responsibilities include administering medications, monitoring conditions, and documenting care progress with families and physicians. Travel and independence are key, with opportunities for growth within the Adoration Home Health

Qualifications

• LPN/LPT/LVN degree from an accredited nursing program with current state license.
• CPR certification required.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Provide 1:1 home health nursing care following the plan of care.
• Administer meds, feedings, oxygen, and ostomy care as needed.
• Document nursing actions and communicate changes to care team and family.
• Coordinate supplies and medications to ensure availability.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on handEnsuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
