Job summary

Deer Meadows Rehabilitation Center in Philadelphia seeks an LPN to join our SNF team. You will provide high-quality nursing care to residents and work closely with the interdisciplinary team.

Shifts available 7am-7pm and 7pm-7am; PA licensure required; long-term care experience preferred; responsibilities include patient records and care plans, with compassionate resident support.

Qualifications

• LPN licensure in Pennsylvania is required.
• Prior experience in long-term care is preferred.
• Experience with basic nursing procedures and patient care is desirable.

Responsibilities

• Provide direct patient care in line with facility policies.
• Take vital signs, perform wound care and other basic nursing tasks.
• Maintain accurate, timely documentation of patient care and treatments.
• Collaborate with the care team to develop and implement individualized care plans.
• Offer emotional support to residents and their families.

About the Role of LPN for SNF:

As an LPN with Deer Meadows, you will play a vital role in our Skilled Nursing Facility (SNF) team, providing high-quality patient care to our residents.

LPN for SNF Responsibilities:

• Providing direct patient care in accordance with our facility's policies and procedures
• Conducting vital signs, wound care, and other basic nursing procedures
• Maintaining accurate and timely records of patient care and treatments
• Collaborating with the interdisciplinary care team to develop and implement individualized care plans
• Providing emotional support and comfort to our residents and their families
