Deer Meadows Personal Care Home in Philadelphia, PA is seeking an LPN to join our compassionate team for evening shifts three days a week, with a night shift Mon-Thu and day shift on alternating weekends available.

You will deliver high-quality nursing care, develop care plans with the care team, monitor residents, educate families, and maintain accurate records.

Qualifications

About the LPN for Personal Care Role

As a Licensed Practical Nurse (LPN), you will be responsible for:

• Providing high-quality nursing care to residents in our Personal Care Home,

• Monitoring and reporting changes in resident condition, including any concerns or issues that may arise




• Educating residents, families, and caregivers on care instructions

LPN for Personal Care Requirements

• Prior experience in a healthcare setting
