Job Title: LPN / Licensed Practical Nurse - $10,000 Sign-On Bonus

Pay Range: USD $30.00/Hr.

Position Overview

Under the supervision of a Registered Nurse, the LPN provides routine care, observes clients' health, assists with treatments, and communicates with clients and their families. Reports changes in condition to supervisors.

Responsibilities

1. Communicate health/nursing needs and recommendations to RN supervisor.
2. Coordinate admissions, discharges, and follow-up care.
3. Assist in medical appointments and ensure provision of nursing services as per care plans.
4. Take and record vital signs, weights, wound measurements, and identify trends outside normal parameters.
5. Report health status changes to physicians and team members promptly.
6. Administer medications correctly, monitor responses, and document appropriately.
7. Ensure availability of medical supplies and maintain medication records.
8. Report illnesses and infections, and maintain a safe environment.
9. Provide supervision and education to non-licensed staff.
10. Participate in quality improvement activities and team meetings.
11. Work hours may vary, including weekends and evenings.

Qualifications

• Completion of a state-approved training program (diploma, associate's degree, etc.).
• Licensed Practical Nurse (LPN) in good standing per state licensure.
• CPR certification.
• At least 6 months of direct nursing experience.
• Experience with individuals with developmental disabilities preferred.
• Valid driver’s license with a satisfactory driving record.
• Proficiency with computer documentation systems.
• Effective communication skills both verbally and in writing.

Additional Information

• Preference for candidates with experience in developmental disabilities.
• Some travel required (must have active Indiana driver’s license).
• Positions available both full-time and part-time.

Salary

USD $30.00 per hour
