Job summary

Interim HealthCare of Augusta in Virginia Beach and surrounding areas is hiring Home Health LPNs to provide compassionate, personalized care in patients' homes under the supervision of an RN.

You will take vitals, perform assessments, document progress, teach self-care, educate families, and coordinate with an interdisciplinary team; a Virginia LPN license, CPR certification, and at least 2 years of LPN experience are required.

Qualifications

• CPR certification required.
• Virginia LPN license active.
• Minimum 2 years' LPN experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing with strong communication and compassion.

Responsibilities

• Provide home-based care to patients under supervision of an RN.
• Work with physicians, RNs, CNAs and aides to deliver quality care.
• Take patient vitals, perform assessments and document progress.
• Assist with plan of care and teach patient self-care techniques.
• Educate family on plan of care, medications and home safety.
• Collaborate with team to coordinate care and discharge planning.

As a Home Health LPN, here's a big-picture view of what you'll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases - often after hospital discharge - under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient's plan of care and teaching patient self-care techniques
• Educate family on the patient's plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Licensed Practical Nurses:

• Graduate of an accredited practical nursing program and active LPN license in Virginia
• Minimum of 2 years' LPN experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
