Job summary

Human Touch Home Care is hiring Licensed Practical Vocational Nurses in Colorado Springs for direct patient care under a registered nurse. Follow the plan of care, provide treatments, and collaborate with the team to improve patient outcomes.

Pay is per visit ($30.00-45.00; typical visit ~1 hour). Full-time roles include comprehensive benefits such as health, vision, dental, PTO, sick leave, and retirement plans in a supportive home health environment.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication and organizational skills.
• Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
