Job summary

Creekside Center for Rehabilitation and Healing in Tennessee seeks an LPN (Licensed Practical Nurse) to join our post-acute care team. We offer top pay with options for weekly and daily pay, and shifts include day, night, full-time, part-time, and per diem.

Duties include medication administration, admissions/discharges, care documentation, resident education, infection control, and collaboration with the physician and interdisciplinary team to ensure high-quality care.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Graduate from approved LPN or RN program.

Responsibilities

• Administer medications and treatments.
• Assist with admissions, transfers and discharges.
• Chart resident care in medical records.
• Educate residents and families on treatment and status.
• Follow infection control and facility policies.
• Consult with physicians about plan of care.
• Maintain confidentiality and privacy.
• Support DOH and facility standards of care.
• Collaborate with interdisciplinary team to solve problems.

Salary: Up to $33 an hour

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
