Job summary

Creekside Center for Rehabilitation and Healing is seeking an LPN to provide post-acute care in a facility with therapies across physical, occupational, and speech services. The role offers top pay and weekly pay with daily pay options, and shifts include day, night, full-time, part-time, and per diem.

The position requires a nursing degree (LPN or RN) and commitments to infection control and resident privacy.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Administer medications and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care in medical records and department records.
• Educate residents and families about treatment and rehabilitation.
• Adhere to infection control and facility policies.
• Consult with physician regarding plan of care and changes.
• Maintain patient confidentiality.
• Ensure quality of care per DOH standards.
• Collaborate with interdisciplinary team to solve resident issues.

Salary: Up to $33 an hour

Available Shifts: Day shift, Night Shift, Full time, Part time, Per Diem.

Duties Include:

• Medication administration and treatments.
• Assist with admission, transfer and discharging of residents
• Document resident care services by charting in resident medical record and department records.
• Educate residents regarding treatment, rehabilitation, change of status, etc… as well as family members.
• Adhering to infection control policies and procedures as well as all other facility policies and procedures.
• Consult with resident’s physician regarding resident’s plan of care as well as notifying them of any changes.
• Maintain resident’s confidentiality and privacy.
• Assure quality of care by adhering to DOH standards of practice and facility standards of care.
• Assist with solving resident’s problems by working with the interdisciplinary team.

REQUIREMENTS/QUALIFICATIONS:

Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.
