Position Summary
As an LPN, you will provide resident care under the medical direction and supervision of attending physicians, the Director of Nursing Services, and the facility’s Medical Director. This role includes medication administration, treatments, documentation, resident monitoring, and support of individualized care plans that meet each resident’s physical and psychosocial needs.
Key Responsibilities
- Prepare, administer, and document medications accurately and safely in accordance with physician orders and the six rights of medication administration.
- Administer medications and treatments through appropriate routes, including oral, IM, subcutaneous, G-tube, and IV fluids or medications under RN supervision when applicable.
- Document medication administration, physician orders, nurses’ notes, and resident care using the electronic medical record.
- Monitor resident conditions and report changes, incidents, medication errors, possible abuse, accidents, near misses, or safety concerns in a timely and appropriate manner.
- Follow resident care plans and participate in the care planning process as needed.
- Provide routine nursing care, including skin care, dressing changes, ostomy care, catheter care, tube feedings, respiratory services, oxygen monitoring, and nebulizer treatments.
- Use nursing equipment safely and appropriately, including glucometers, PT/INR machines, bladder scanners, and lifts, as applicable.
- Make rounds throughout the shift, communicate shift reports, assist with admissions and discharges, and make rounds with physicians as assigned.
- Practice safe infection control standards, follow emergency procedures, and report exposure to blood-borne pathogens or hazardous chemicals as required.
- Honor resident choice and support residents in directing their own care, daily schedules, activities, and personal routines.
- Participate in required educational programs and in-service training.
- Reflect the mission, vision, values, and customer service standards of Lutheran Services Carolinas in daily interactions.
- Perform other duties as assigned.
Supervisory Responsibilities
When applicable, this role oversees members of the nursing team, including CNAs, restorative nursing assistants, and medication aides.
Qualifications
- Nursing degree from an accredited college or university or graduation from an approved LPN program
- Current Licensed Practical Nurse license in the state of North Carolina
- CPR certification or ability to become CPR certified
- Ability to remain cordial and tactful while communicating with residents, families, peers, and others
- Commitment to maintaining confidentiality
Preferred Qualifications
- Two years of LPN experience preferred
- Computer skills preferredAbility or willingness to learn electronic resident documentation
Working Conditions and Physical Requirements
- Physical exertion involving pushing, pulling, and lifting up to 35 pounds, with occasional lifting up to 50 pounds
- Prolonged standing and/or walking while transporting objects
- Occasional bending, squatting, or repeated reaching
- Ability to assist with repositioning and transferring individuals of varying weights while following established safe lifting procedures
- Ability to ambulate throughout the facility
