Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. In this role, you will provide direct nursing care to residents, support rehabilitation goals, and collaborate with RNs, physicians, and therapists to deliver the highest standards of care.

Responsibilities
  1. Administer prescribed medications and treatments.
  2. Monitor, document, and report residents’ health status and vital signs.
  3. Assist residents with daily living activities (ADLs), including bathing, dressing, and feeding.
  4. Collaborate with therapy teams (PT, OT, ST) to support recovery goals.
  5. Maintain accurate, timely, and compliant documentation.
  6. Provide education and emotional support to residents and families.
  7. Ensure a safe, clean, and compliant care environment.
Qualifications
  1. Current and valid LPN license.
  2. Strong clinical skills (including medication administration and wound care).
  3. Excellent communication and interpersonal skills.
  4. Ability to thrive in a team-oriented environment.
  5. Compassionate, resident-centered approach to care.
  6. BLS/CPR certification required.
