Job summary

Stone Bridge Center for Health & Rehabilitation in Sandy Hook, CT is seeking a Licensed Practical Nurse (LPN) for a 3-11pm shift. You will provide direct nursing care, administer medications, monitor residents, and collaborate with a multi-disciplinary team to implement care plans.

Qualifications

• Must have a valid state LPN license.
• Compassionate and empathetic approach to patient care.

• Strong clinical skills including administration of medications.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively in a multidisciplinary team environment.

Responsibilities

• Administer medications and treatments as prescribed.

• Supervising and supporting Certified Nurse Assistants (CNAs) in delivering care
