Job summary

Life Care Centers of America in the Mesa, AZ area seeks a Licensed Practical Nurse to join our interdisciplinary team delivering compassionate nursing care in a skilled nursing facility setting.

You will monitor patients, administer medications, coordinate with therapists and CNAs, and help residents achieve optimal functional well-being while adhering to federal, state, and Life Care standards.

Qualifications

• Graduated from an accredited LPN program.
• Currently licensed/registered in applicable State with an active LPN license.
• One year skilled nursing experience preferred.
• CPR certification upon hire or during orientation; CPR must remain current.

Responsibilities

• Deliver quality nursing care to patients and monitor changes in condition.
• Instruct and provide clinical supervision to CNAs; coordinate with interdisciplinary team.
• Maintain patient well-being in accordance with Life Care standards and regulations.

As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards. Your role will involve monitoring and reporting changes in the patient’s condition. On a daily basis you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

Education, Experience, And Licensure Requirements

• Graduated from an accredited LPN program
• Currently licensed/registered in applicable State. Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
• One (1) year skilled nursing experience preferred
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
