Yale New Haven Health is seeking an LPN to join our home health team in Connecticut. Under the supervision of an RN, you will provide nursing care, support patient self-care learning, and help prepare clinical and progress notes during each home visit.
The ideal candidate holds a CT Licensed Practical Nurse license with CPR certification, a valid driver's license, and at least one year of acute care experience.

The Licensed Practical Nurse works under the direction of a registered nurse to provide nursing care, to assist the patient in learning self-care techniques and to prepare clinical and progress notes.

Graduated from an approved Practical Nurses Education Program
Minimum one year experience in the acute care setting. Home care experience preferred, but not required.
License Practical Nurse with valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
Excellent verbal and written skills. Excellent clinical knowledge. Strong computer skills.
Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
