Job summary

Greenhills Health and Rehabilitation Center is seeking a skilled Occupational Therapist to join our care team. You will assess patients in need of rehabilitation, develop personalized treatment plans, and work under physician supervision to restore function.

The role emphasizes patient-centered care, documentation accuracy, and collaboration with therapists and nurses. Prior geriatric or long-term care experience is a plus; punctuality and professionalism are essential.

Qualifications

• Must be a graduate of an accredited OT program.
• Must hold valid OT license and state registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care and/or geriatric care is preferred.
• Strong documentation skills.
• Ability to perform evaluations, document progress notes, and discharge planning.

Responsibilities

• Assess referred patients to determine rehabilitation needs and goals.
• Develop and administer treatment programs with specific goals based on patient capacity and tolerance.
• Set realistic goals, document patient condition, and educate patients and families.
• Oversee and supervise Occupational Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care and maintain confidentiality and HIPAA compliance.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
