Job summary

Greenhills Health and Rehabilitation Center is seeking a dedicated Occupational Therapist to assess referred patients, develop and manage treatment programs, and collaborate with a multidisciplinary team to achieve patient-centered outcomes.

You will set achievable goals, document progress, educate patients and families, and ensure compliance with confidentiality and regulatory standards in a long-term care setting.

Qualifications

• Graduated from an accredited occupational therapy program.
• Must hold a valid OT license and registration for state of practice.
• Oversees and supervises occupational therapy assistants.
• Experience in long-term or geriatric care preferred.
• Strong documentation writing skills and charting.

Responsibilities

• Assess referred patients requiring rehabilitation and plan care.
• Provide direct patient care to evaluate medical condition and functional capacity.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Set realistic goals with patients and families and document progress.
• Educate patients and families on therapy methods and goals.
• Collaborate with all disciplines to deliver patient-centered care.
• Ensure confidentiality and compliance with HIPAA and Residents Rights.

Occupational Therapist

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working OccupationalTherapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
