Job summary

Life Care Center of Evergreen is seeking a licensed practical nurse to join our caring, five‑star team west of Denver. You will deliver quality nursing care, monitor patient status, and support CNAs within Life Care standards.

We value teamwork, compassionate service to residents, and opportunities for growth. CPR certification and an active LPN license are required; prior SNF experience is a plus. Enjoy a supportive environment with opportunities for shift differentials.

Qualifications

• Graduated from an accredited LPN program.
• Currently licensed/registered in applicable State. Must maintain an active LPN/LVN license in good standing throughout employment.
• One (1) year skilled nursing experience preferred.
• CPR certification upon hire or during orientation; CPR certification must remain current during employment.

Responsibilities

• Deliver quality nursing care to patients through interpersonal contact.
• Monitor and report changes in the patient’s condition.
• Assign duties, instruct, and provide clinical supervision to CNAs.

As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards. Your role will involve monitoring and reporting changes in the patient’s condition. On a daily basis you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

Education, Experience, And Licensure Requirements
