Job summary

Sylvania Health Care Center is seeking a compassionate LPN to join our care team. The role emphasizes patient comfort, adherence to care plans, and collaboration with CNAs and CMTs to maintain high-quality resident care.

Responsibilities include admissions, ongoing assessments, wound care, and emergency response, with a focus on maintaining patient dignity and safety within our facility.

Qualifications

• Ability to follow protocols and document changes accurately.
• Willingness to lead and coordinate care with CNAs and CMTs.

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
