Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team. The SLP will assess patients requiring rehabilitation, provide direct patient care, and develop treatment plans aligned with patient capacity and physician direction.

Responsibilities include documenting patient progress, educating families, and collaborating with an interdisciplinary team to deliver patient-centered care in a long-term care setting.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid TN state license and registration.
• Experience in long-term care or geriatric settings preferred.
• Proficient in patient documentation and progress notes.
• Collaborates with interdisciplinary team to enhance patient care.
• Maintains confidentiality and adheres to HIPAA and Residents Rights.

Responsibilities

• Assess referred patients needing rehabilitation and provide direct care.
• Develop and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
