Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team in Westmoreland, TN. The role includes evaluating patients, developing individualized treatment plans, and delivering direct therapy within a multidisciplinary rehab team.

The SLP will document patient progress, communicate with families, and ensure compliance with HIPAA and regulatory standards. A sign-on bonus is offered for qualified candidates, and experience in geriatric care is

Qualifications

• Licensed in Tennessee (or eligible) with valid SLP license.
• Experience in long-term care/geriatric rehab preferred.
• Strong documentation and report-writing skills.
• Collaborates with multidisciplinary team to provide patient-centered care.

Responsibilities

• Assess referred patients for rehabilitation needs and create treatment plans.
• Provide direct patient care and document patient progress and discharge plans.
• Educate patients and families on therapy goals and strategies.
• Coordinate with physicians and rehab staff to optimize patient outcomes.

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team.

Speech Language Pathologist (SLP) Responsibilities Include:

The Speech Language Pathologist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Speech Language Pathologist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Speech Language Pathologist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Speech Language Pathologist (SLP) Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
