Responsibilities

• Provide direct patient care to a diverse population of residents, including those with complex medical needs and conditions
• Assess, plan, implement, and evaluate individual patient care plans in collaboration with the multidisciplinary care team
• Administer medications, treatments, and therapies as prescribed by physicians and as outlined in patient care plans
• Monitor patient responses to treatments and report any concerns or adverse reactions to the healthcare team
• Maintain accurate and detailed patient records, including medication administration records and treatment notes
