Job summary

Mattapan Health & Rehabilitation Center in the Boston area seeks an experienced Licensed Practical Nurse to lead daily clinical care, coordinate with physicians and a multidisciplinary team, and ensure compassionate, high-quality resident care in a safe environment.

You will administer medications, monitor residents, supervise CNAs, and contribute to care plans and documentation to support optimal resident outcomes and regulatory compliance.

Qualifications

  • Active LPN license in MA or applicable state.
  • Graduate of an accredited Practical Nursing program.
  • Long-term care or skilled nursing experience preferred but not required.

Responsibilities

  • Administer medications and treatments according to physician orders and nursing standards.
  • Monitor residents for changes in condition and communicate concerns to physicians and team.
  • Supervise and support CNAs; promote teamwork and quality resident care.
  • Assist with direct resident care as needed.
  • Implement and monitor individualized care plans for person-centered care.
  • Complete accurate clinical documentation (medication records, treatment records, nursing notes).
  • Safely manage controlled medications per policies and regulations.
  • Participate in rounds, rounds, and discharge planning as needed.
  • Maintain infection prevention and quality assurance practices; ensure compliance with regulations.
  • Collaborate with rehab, dietary, social services, and nursing teams.

Responsibilities

What You'll Do

As a Licensed Practical Nurse, you'll help lead the daily clinical care of residents while ensuring the highest standards of quality, safety, and compassion.

Responsibilities Include
  • Administer medications and treatments according to physician orders and established nursing standards.
  • Monitor residents for changes in condition and promptly communicate concerns to physicians, Registered Nurses, and interdisciplinary team members.
  • Supervise and support Certified Nursing Assistants while promoting teamwork and quality resident care.
  • Assist with direct resident care as needed.
  • Implement and monitor individualized care plans to ensure residents receive person-centered care.
  • Complete accurate and timely clinical documentation, including medication administration records, treatment records, nursing notes, and physician orders.
  • Safely manage controlled medications in accordance with facility policies and regulatory requirements.
  • Participate in physician rounds, care conferences, admissions, transfers, and discharge planning.
  • Maintain compliance with infection prevention practices, quality assurance initiatives, and state and federal regulations.
  • Collaborate with rehabilitation, dietary, social services, and nursing teams to promote positive resident outcomes.

Qualifications
What We're Looking For
  • Current, active Licensed Practical Nurse (LPN) license in the applicable state.
  • Graduate of an accredited Practical Nursing program.
  • Long-term care, rehabilitation, or skilled nursing experience is preferred but not required.
  • Strong clinical judgment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to lead and mentor Certified Nursing Assistants while fostering a collaborative team environment.
  • Commitment to compassionate, resident-centered care.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
