As a Phoenix Home Health LPN, you will be the point of contact between patients and medical providers. The LPN should possess strong skills in physical assessment and communication to complete the individualized plan of care according to physician orders, which includes analysis of initial assessments. You will work alongside other nurses, Physical Therapists, Speech Therapists, and Occupational Therapists.

Responsibilities:

• Assist in review and revision of the plan of care
• Provide nursing care in accordance with physician's plan of treatment and individual plan of care, as authorized by the client's payer source
• Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physicians
• Provide ongoing instruction to clients or families in the plan of treatment
• Participate in education programs, quality assessment activities, nurses' meetings, and other staff meetings as required

Requirements:

• Missouri or Compact Licensed Practical Nursing License
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver's license
