Job summary

Hiawatha Care Center is actively hiring a Licensed Practical Nurse (LPN) to provide clinical leadership and skilled nursing care to residents. The LPN collaborates with nursing staff, providers, residents, families, and interdisciplinary teams to ensure high-quality resident outcomes.

We offer Full-Time Days (6AM-6PM) and Full-Time/Nights (6PM-6AM), plus Part-Time Nights (6PM-10PM), with every other weekend. Current Iowa LPN license and BLS/CPR are required, with weekend bonus eligibility.

Qualifications

• Current Iowa LPN license.
• Current BLS/CPR certification.
• Previous LPN experience preferred.
• Strong clinical assessment and critical thinking.
• Ability to supervise and support nursing staff.
• Excellent communication and leadership abilities.
• Willing to complete Long-Term Care Supervisors Course if required by state regulations.

Responsibilities

• Provide skilled nursing care and clinical oversight as an LPN.
• Assess resident conditions and respond to changes in health status.
• Develop, implement, and evaluate individualized care plans.
• Administer medications and treatments per physician orders.
• Communicate with residents, families, providers, and inter-disciplinary team members.
• Order medications and coordinate resident healthcare needs.
• Oversee resident admissions, discharges, and transfers.
• Coordinate resident appointments and transportation arrangements as needed.
• Supervise, support, and mentor CNAs.
• Ensure documentation is accurate, timely, and regulatory compliant.
• Support infection control, quality assurance, and resident safety initiatives.
• Promote a positive team culture through leadership and collaboration.
