Job summary

Starling in Florida is seeking a Licensed Practical Nurse to provide leadership and oversight of resident health and wellbeing in the community.

The role covers clinical care, medication distribution under state regulations, collaboration with providers and administration, and ensuring compliance with laws. Requires Florida LPN license, 2 years in LTC, assisted living, or home health; strong assessment, injections, charting, triage and team leadership.

Qualifications

• Maintains current LPN license as required by state regulations.
• Experience in assisted living, home health, or long-term care.
• Two years experience as LPN.
• Competence in assessment, injections/medication administration, charting, triage, communication, and team leading.

Responsibilities

• Maintains communications with Healthcare Providers of recent changes or requests for orders.
• Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Responsible for collecting, and reporting occurrences, and if necessary, assist in developing/implementing improvement plans.
• Partners with Administrator and other team members to maintain resident and team member health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when floor nurse is not available.
• Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.
• Responsible for the completion of any assigned evaluations.
• Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Mantains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Other duties as assigned.
