Job summary

Montefiore Medical Group is seeking an experienced LPN to join a collaborative care team. You will provide hands-on nursing support under RN and physician supervision, administer medications, monitor vitals, and assist with treatments in a fast-paced clinical setting.

Responsibilities include accurate documentation, room turnover, and patient education. A NY LPN license, BLS, and infection control certification are required. This is a full-time, weekday schedule supporting patient-centered care.

Responsibilities

• Provide hands‑on care under RN/physician supervision, including administering medications, monitoring vitals, wound care, triaging/rooming patients within 15 minutes, and performing treatments.
• Accurately document patient info, vitals, interventions, and medication administration with 100% compliance. Collect, label, and log specimens (blood, biopsy) with zero tolerance for error.
• Clean and stock exam rooms between sessions, maintain supply par levels, and sterilize equipment via autoclave.
• Partner with the healthcare team to elevate issues, facilitate discharge, and teach patients/families regarding care management.
• Provides direct care for a specific patient population. Administers medications and treatments. Provides patient & family teaching and discharge instructions.

Requirements

• NY LPN License & Registration.
• BLS Card and Infection Control Certification.
• High School/GED minimum.

Shift: Monday to Friday 0930A - 0600P

Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full‑time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full‑time employees, and four (4) months for part‑time employees who works more than 3/5 of the normal workweek.
