JOB SUMMARY

Responsible for receiving and processing referrals, coordinating intake activities, obtaining required documentation, communicating referral information to the clinical team, coordinating admissions and resumption of care services, and supporting patient outreach, care coordination, quality improvement, and other agency operations as assigned. Reliable transportation, a current valid Arkansas driver's license, and automobile liability insurance are required as a condition of hire and must be maintained throughout employment. Limited amount of is travel is required.
