Job summary

Louisiana HomeCare of Raceland, part of the Optum family, is seeking an Intake Coordinator-LPN to join our team. You will receive and analyze referrals, coordinate with clinical and marketing staff, and ensure accurate patient information for seamless care delivery.

Responsibilities include obtaining demographic and clinical data, verifying physician licenses, researching coverage, and educating patients and hospitals about home health services.

Qualifications

• 1+ years of experience in office procedures.
• 1+ years of insurance/medical experience.

Responsibilities

• Obtain demographic and clinical information from clients, physician’s offices, and other referral sources to compile referrals.
• Verify new referring physician licenses on referrals taken in department.
• Research coverage or pay source for each referral and verifies commercial policies and HIQA.
• Educate prospective patients, families, physicians, and hospitals regarding home health services available.
• Serve as a liaison between clinical and marketing staff.
