Job summary

You will manage referrals via Home Care Home Base and liaise with referral sources to ensure timely, quality patient onboarding.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state license as an LPN.
• Valid State Driver's License.
• One year experience as an LPN, home health preferred.
• Proof of hepatitis profile.
