Job summary

Responsibilities

• Contribute to the plan of care and monitor care.
• Provide nursing interventions safely and report findings to RN/MD.
• Evaluate patient response to care and adjust plan with RN/MD.
• Address age-specific care needs and communicate findings to RN.
• Coordinate care with the health care team.
• Orient patients to services and educate per care plan.
• Assist with routine health maintenance and follow-up care.
• Document data accurately in medical records.
• Maintain a safe environment and adhere to safety standards.
