Job summary

Talented Medical Solutions seeks a dedicated Licensed Practical Nurse (LPN) for a 13-week SNF assignment with a guaranteed 40 hours per week and flexible day/evening shifts. You will provide direct patient care under supervision, administer medications, monitor conditions, and support ADLs within a collaborative care team.

Active LPN license in the assignment state is required, with at least 1 year of recent LPN experience (SNF/long-term care preferred) and BLS certification.

Qualifications

• Active LPN license in state of assignment.
• Minimum of 1 year of recent LPN experience, preferably in a SNF or long-term care setting.
• BLS certification (AHA preferred).
• Strong communication and organizational skills.
• Ability to work flexible shifts as scheduled.

Responsibilities

• Deliver direct patient care in accordance with the plan of care under the supervision of an RN or physician.
• Administer medications and treatments as prescribed.
• Monitor and document patient conditions and report changes to the healthcare team.
• Provide support with activities of daily living (ADLs).
• Collaborate with interdisciplinary team members to ensure quality patient care.
