Job summary

Pleasant Acres Nursing & Rehabilitation Center in Pennsylvania is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. You will administer medications, monitor residents, and participate in care planning in a fast-paced environment.

You will work with an interdisciplinary team to deliver high-quality resident care, maintain accurate records, and communicate changes in condition to the RN and other professionals. A PA LPN license is required.

Qualifications

• Current LPN licensure in the State of Pennsylvania.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team environment.

Responsibilities

• Provide skilled nursing care to residents, including administering medications, performing vital signs, and documenting patient data.
• Participate in care planning with the interdisciplinary team to develop and implement individualized care plans for each resident.
• Observe and report changes in residents' conditions to the Registered Nurse and other healthcare professionals.
• Collaborate with other healthcare professionals to ensure the delivery of high-quality care.
• Maintain accurate and up-to-date records of patient care and treatment.
