Job summary

Centra Health’s 26-bed Long Term Care Hospital unit in Central Virginia seeks an LPN to deliver patient care under supervision. The unit specializes in medically complex, extended hospitalization with advanced clinical care, including dialysis bedside treatments using TABLO machines.

As part of an interdisciplinary team, the LPN coordinates and manages care for patients of all ages, upholds Centra's quality standards, and maintains a safe patient environment to achieve optimal outcomes.

Responsibilities

• Provide patient care under the direction or supervision of a licensed medical practitioner, a professional registered nurse, or other licensed health professional, as authorized by the Health Regulatory Boards.
• The LPN coordinates and manages the care provided to assigned patients of all ages based on the unit care delivery model.
• Collaborates with the interdisciplinary team to deliver high-quality patient care, adheres to Centra’s best practices for quality and service, creates and maintains a safe patient environment, and coordinates various care processes that are necessary to achieve desired outcomes.

• Patient Population:Specialize in caring for medically complex patients who require extended hospitalization and advanced clinical care.
• Typical Cases:Care for individuals recovering from critical illness who need ongoing respiratory support, complex wound care, intravenous therapies, cardiac monitoring, dialysis, and comprehensive rehabilitation services before transitioning to a lower level of care.
• Additional Info:Weutilizethe LEAF system, a wearable patient monitoring technology that helps staff track and document patient repositioning.Our unit provides dialysis treatments at the bedside using TABLO dialysis machines. This innovative technology allows patients to receive dialysis without leaving their room, improving comfort and continuity of care.
