Centra Health is seeking anLPN to provide patient care under supervision of licensed practitioners in a Virginia facility. You will perform assessments, administer medications, and collaborate with the care team to ensure safe patient outcomes.

The role requires an accredited LPN program completion, Virginia license, and BLS certification. Experience preferred; compensation is commensurate with local market rates and the posted hourly range above.

Formación

• Completion of accredited LPN training program.
• At least one year of Nursing (LPN) experience.
• BLS Certification.
• A valid, current, full and unrestricted LPN license in the State of Virginia.

Responsabilidades

• Conduct initial assessment in collaboration with RN to co-sign.
• Conduct ongoing individualized patient assessments as determined by patient condition and policies.
• Integrates nursing interventions into the multidisciplinary plan of care.
• Performs appropriate treatments as ordered by provider in an accurate and timely manner.
• Provides medication administration following Centra policy for scope of practice.
• Evaluates patient’s responses to care and updates the care plan as needed.
