Job summary

Qualifications

• High School Equivalency Diploma.
• LPN license active and in good standing in AZ.
• Basic Life Support (BLS) certification.

Responsibilities

• Inspires hope and nurtures the well-being of the whole person, respecting physical, emotional and spiritual needs.
• Listens attentively to patients, visitors and colleagues to ensure understanding of their needs; delivers personalized service and maintains a safe environment.
• Collects data and provides input to the RN/health care team to assist in developing an individualized plan of care.
• Observes, recognizes, and reports information/changes related to the patient’s needs and takes action as appropriate.
• Collects data and communicates information to assist in patient assessment and care planning.
• Reports and documents appropriate information related to the patient/family.
• Verifies that patient charts are updated promptly and accurately.
• Delivers care in accordance with the patient care plan and organizational policies.
• Performs other duties as assigned.
