Benchmark Human Services seeks an LPN House Manager for its Warner Robins IDD Crisis Respite Home. The role supervises direct care staff, coordinates medical services, and ensures health and safety for 10–17-year-olds with developmental disabilities.

The LPN must hold a Georgia nursing license, CPR/First Aid, and crisis intervention training, with at least two years in crisis settings and a valid driver’s license. Weekend/on-call coverage is required.

Qualifications

• Active Georgia nursing license with clinical supervision.
• Crisis intervention certification or training.
• Current CPR and First Aid certification.
• Minimum 2 years of experience with individuals in crisis, including IDD or mental illness.

Responsibilities

• Monitor and supervise health care services and staff.
• Schedule and supervise direct care staff and CNAs.
• Liaise with physicians, hospitals, and other clinicians to coordinate care.
• Provide ongoing health care training and ensure regulatory compliance.
• Maintain medical records and ensure documentation is current.
• Respond to on-call needs for crisis program.

Our Warner Robins, adolescent crisis stabilization services are looking to add a Licensed Practical Nurse (LPN) House Manager to serve as a member of the Intellectual and Developmental Disabilities (IDD) Respite Home team. The LPN will serve as the home manager who is responsible for staff supervision, health and safety of individuals in the home and coordinate and provide services. The focus of this program is to provide time-limited crisis services that support kids and adolescents ages 10-17 yrs old with developmental disabilities.

The adolescent IDD Crisis Respite Home is used as a measure of last resort for individuals undergoing an acute crisis. The goal is to stabilize the individual through nursing and behavioral supports, on a time-limited basis and then discharge them back to a permanent home placement. Our services are with the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD).

Job Responsibilities:

• Provide direct monitoring, assessment, and staff training of health care services including, specific interventions, treatment protocols, improvement of health care outcomes, etc.
• Supervise and schedules direct support staff and CNAs.
• Function as a liaison and advocate for the individuals when dealing with physicians, psychiatrists, hospitals, and consultants including RD, OT, PT, SLP, etc.
• Provide on-going health care training and monitoring of Benchmark staff as required by state and federal regulations, and Benchmark Crisis Assistance policies and procedures.
• Uses positive behavior support strategies as described in behavior support plans or behavior guidelines.
• Responsible for maintaining prior authorizations or obtaining medications and treatments covered by client’s insurance plan or approved contractors.
• Responsible for scheduling and responding to quarterly pharmacy reviews.
• Complete daily, weekly, monthly, quarterly and yearly paperwork and assessments of clients’ health care needs
• Provide guidance during client emergency medical situations.
• Monitor and maintain clients’ medical file to ensure that all documentation is current.
• Facilitate communication between all medical services providers, IDT, LSCW, and supervisor to ensure quality of care.
• Attend all agency, departmental, and client specific meetings, including client appointments as directed.

Qualifications:

• Valid unrestricted Georgia Nursing License to practice nursing in the state of GA under the clinical supervision of a Registered Nurse.
• Certification and ongoing training in crisis intervention curriculum.
• Valid CPR and First Aid Certification
• 2 years experience working with individuals with intellectual and/or developmental disabilities and/or mental illness who are in crisis.
• Valid Driver’s License and auto insurance.
• Prefer one-year supervisory experience
• Must be computer literate.
