Corewell Health South Homecare provides high-quality in-home care to patients requiring short-term assistance with daily activities and specialized treatments, from infants to geriatrics. We address a critical need for affordable community-based healthcare and maintain a high star rating per CMS for quality of care. Nurses are assigned to specific territories to minimize driving and maximize patient interaction. We offer flexible scheduling, training, and a rewarding career caring for patients in their homes.

About Corewell Health South - Lakeland Hospitals

The health system includes three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 physician practice locations. We provide inpatient and outpatient medical services throughout Southwest Michigan.

Scope of work

The RN, within the ANA scope of practice, uses the nursing process to provide professional care, incorporating patient-family-centered care principles to assess, plan, implement, evaluate, and discharge. The RN delivers quality patient care under the direction of the Nurse Leader, following Corewell Health policies and recognizing the impact of nursing care and patient/family partnerships on outcomes. The RN collaborates with patients and families in planning care and develops educational strategies, documenting education appropriately in health records.

Qualifications

Required

  • Associates in Nursing
  • Graduates from a non-accredited nursing school must have at least one year of acute care experience or two years in a licensed healthcare role, and must have passed the NCLEX exam.
  • Licensed Practical Nurse (LPN) registered in Michigan upon hire
  • Valid MI Driver's License
  • One year of recent clinical experience in home health or similar setting
