Position Summary
We have an exciting opportunity to join our team as an LPN - Homecare, Nassau County *Visiting Nurse/ Multiple patients a day*.
In this role, the successful candidate is responsible for using their clinical experience to plan and deliver safe, therapeutic, age-appropriate patient care through planning, implementation, and evaluation within their scope of practice. The LPN works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes. Performs multiple clinical and clerical duties as required, adhering to hospital policies and federal, state, and local regulations, including HIPAA guidelines. Performs all other duties related to the specialty.
High School or Equivalent Technical Diploma, graduate of an accredited LPN program, and valid LPN license. 0-1 year nursing experience preferred, in hospital, outpatient, or home health settings.
Required Licenses: LPN

Salary range: $72,907.17 - $84,207.84 annually, depending on experience and qualifications. This range complies with New York State salary transparency laws.
