Position Summary
We have an exciting opportunity to join our team as an LPN - Homecare, Nassau County *Visiting Nurse/ Multiple patients a day*.

In this role, the successful candidate is responsible for using their clinical experience to plan and deliver safe, therapeutic, age-appropriate patient care through planning, implementation, and evaluation within their scope of practice. The LPN works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes. Performs multiple clinical and clerical duties as required, adhering to hospital policies and federal, state, and local regulations, including HIPAA guidelines. Performs all other duties related to the specialty.

Job Responsibilities
  • Perform other duties as assigned.
  • Complete documentation reflecting multidisciplinary coordination. Document teaching/learning needs of patients and families, and evaluate patient responses to interventions. Address clinical issues promptly and escalate as needed. Participate in risk management and quality care initiatives. Communicate effectively with patients and team members, including family-centered care principles. Maintain a safe environment, respond to emergencies, and ensure emergency equipment is operational.
  • Obtain prior authorization for medications and referrals as required by insurance. Maintain cleanliness and supplies in exam rooms. Process medication orders and refills accurately, verifying dosages and documenting appropriately. Address patient satisfaction issues and document advance directives. Establish collaborative relationships with healthcare team members.
Minimum Qualifications

High School or Equivalent Technical Diploma, graduate of an accredited LPN program, and valid LPN license. 0-1 year nursing experience preferred, in hospital, outpatient, or home health settings.

Salary range: $72,907.17 - $84,207.84 annually, depending on experience and qualifications. This range complies with New York State salary transparency laws.
