Job summary

Maxim Healthcare is seeking compassionate Licensed Practical Nurses (LPNs) to provide high-quality home health care for adult and pediatric patients in the Wauwatosa area. The role offers full-time and part-time opportunities with competitive pay and flexible scheduling.

You will deliver nursing care per physician orders, assess health status, document findings, and communicate changes to the care team. Wisconsin LPN licensure and BLS/CPR are required.

Qualifications

• Current and unrestricted LPN license in Wisconsin.
• Current BLS/CPR certification.
• TB test or Chest X-Ray current.
• Previous nursing experience preferred (1 year or more).
• Strong clinical, communication, and documentation skills.

Responsibilities

• Deliver skilled nursing care in accordance with physician orders and care plans.
• Assess, monitor, and document patients' physical and emotional health status.
• Identify changes in patient conditions and communicate findings appropriately.
• Develop and implement individualized care interventions.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Maintain accurate and timely clinical documentation.
