Job summary

Maxim Healthcare Services, Inc. seeks compassionate Licensed Practical Nurses (LPNs) to provide home-based care for adult and pediatric patients in the Fort Atkinson area.

Qualifications

• Current and unrestricted LPN license in Wisconsin.
• Current BLS/CPR certification.
• Current TB test or Chest X-Ray.
• 1 year or more nursing experience preferred.
• Strong clinical, communication, and documentation skills.

Responsibilities

• Deliver skilled nursing care in accordance with physician orders and care plans.
• Assess, monitor, and document patients' physical and emotional health status.
• Identify changes in patient conditions and communicate findings appropriately.
• Develop and implement individualized care interventions.
• Collaborate with patients, families, and healthcare teams to ensure quality care.
• Maintain accurate and timely clinical documentation.
