Job summary

Horizon Home Care & Hospice in Kenosha, WI is seeking an experienced LPN to join our Home Care team. You will deliver skilled nursing services in patients’ homes, coordinate care reflecting home health standards, and document care clearly and thoroughly.

The role emphasizes administering medications, identifying patient needs, and communicating with other health professionals as needed. A Monday–Friday schedule with no weekends/on-call is offered, along with a strong benefits package.

Qualifications

• Degree from an Accredited School of Nursing.
• Current WI LPN license.
• 1 year experience as an LPN in acute care, long term care (Geriatrics), or home care or Hospice setting.

Responsibilities

• Provide skilled nursing services in patients' homes as ordered by the physician.
• Identify patient conditions accurately during each visit.
• Administer ordered medications and monitor reactions.
• Complete all required paperwork accurately and timely.
