Innovive Health is hiring a Licensed Practical Nurse (LPN) for Home Health in Massachusetts. You will provide high-quality nursing care to patients in their homes under RN supervision and collaborate with an interdisciplinary care team.

This full-time role emphasizes compassionate, patient-centered care, ongoing education, and a predictable Monday–Friday schedule to support work-life balance. You will help patients manage health, maintain independence, and improve quality of life at home.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver’s license and reliable transportation.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.

Responsabilités

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and healthcare team members.
• Educate patients and families on disease management, medications, and treatment plans.
• Maintain accurate and timely clinical documentation.
• Support patients with dignity, compassion, and respect.
• Participate in care conferences, team meetings, and ongoing training.
